Pup Shield Warranty for Reasonable Veterinarian Expenses for Accidents for Dogs
This warranty covers reasonable veterinarian expenses up to $1500 that are a result of an accident that your dog may have. This warranty is valid from the date of purchase and as long as your monthly payments of $19.99 are current and paid in full. (initial) ______
To be eligible for reimbursement under this warranty, the following conditions must be met:

The accident must occur during the warranty period.
The dog must be properly secured and not involved in any illegal activity at the time of the

accident.
The veterinarian expenses must be reasonable and directly related to the accident.

To make a claim under this warranty, you must provide the following:

A copy of the original sales receipt from the purchase of the puppy.
A detailed report from a licensed veterinarian outlining the injuries sustained by your dog and the

treatments provided.
Proof of payment for the veterinarian expenses.

Once we receive your claim, we will review it to determine if it meets the conditions for reimbursement under this warranty. If your claim is approved, we will reimburse you for the reasonable veterinarian expenses up to $1500.

This warranty does not cover any expenses related to illnesses or diseases, routine check-ups or preventative care, pre-existing conditions, or any expenses exceeding the $1000 (one thousand dollars and zero cents) limit.

By purchasing this warranty, you agree to abide by its terms and conditions. This warranty is non-transferable and cannot be combined with any other warranties or guarantees.

Foot McGuffin LLC DBA Pup Shield ("Seller") warrants to the original purchaser ("Buyer") that the puppy with microchip number _______________________________ (the "Product") will be covered for certain vet bills as a result of an injury as detailed below.
If, during the Warranty Period, the Buyer's dog suffers an accident that requires veterinary care and that injury is directly caused by an accident, Seller will reimburse the Buyer for reasonable and customary veterinary expenses incurred by the Buyer in treating the dog's injuries up to $1500 (one thousand, five hundred dollars). To be eligible for reimbursement, Buyer must provide Seller with proof of purchase, proof of the accident and the resulting veterinary care, and any other documentation that Seller may reasonably require. Buyer must also follow any instructions provided by Seller regarding the submission of claims for reimbursement.

THIS WARRANTY IS IN LIEU OF ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO THE IMPLIED WARRANTIES OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE. SELLER'S LIABILITY UNDER THIS WARRANTY SHALL BE LIMITED TO THE COST OF THE REASONABLE AND CUSTOMARY VETERINARY EXPENSES INCURRED BY THE BUYER UP TO THE MAXIMUM AMOUNT STATED ABOVE, AND IN NO EVENT SHALL SELLER BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES.

This Warranty is governed by the laws of the state of Ohio and any legal action arising under this Warranty must be brought in the courts of that state.
